Short Fuse and Shotgun Blast are better options.
Usually, yes, SF and SB are better. The only advantages I can see to Unforeseen Setback are the following:
This card might actually slow your opponent down at stories if he's trying to rush you. He could theoretically switch to a more defensive approach, and that might buy you enough time to turn the tables on him.
Also, the Agency is big on Attachment manipulation. Fauborg Marigny suddenly becomes pretty good with the Setback, and Price Manor can fetch the Setback for repeatable (if slow) wounding.
So, yes, the Setback only confers some very specialized advantages, and I'd usually rather have the sure things of Short Fuse and Shotgun Blast.
