Sometimes people (including me) will use him just for kicks. But it is not a card you expect to see often.
No, not that much, but think about this - how often does it happen that you have two won stories and need 1-2 more tokens to get a 3rd and win the game? In that situation, Azathoth is a game winning card.

So, he's got a use. The problem is that you need a huge domain for him and if you draw him early he could be a dead card for a long time and Neutrals can be poor resources until later in the game if you've got more than one faction in your deck. So that's your opportunity cost…
Hes a fun idea you might want to force to work.

For example can clear the board using Azatoth :P, with
The Claret Knight (SoK) on your side this might be quite nice.

Using Broken Space, Broken Time (CoC) can make him cheaper, and Called by Azathoth (TSotS) can buy you more time if opponent doesn't run support destruction.

Or you can use ST-Cthulhu, Uroborus-ritual and use ST bouncing effects to not lose the game after he enters play (destroying combo pieces :P).

6 cost neutral card with no icons saying "you lose the game" is not something easy to use reasonably in competitive play, but he can be a nice fun deck base or deckbuilding exercise.
