The effect triggers before the struggels are resolved.
And also before the opponent commits his charactersto the story, if I play this guy on my turn? (So the effect will not affect my opponents characters, right?)
He triggers as soon as he commits. So if it is your turn and your opponent's characters are not committed yet then they would not be affected. So, in that case you wouldn't trigger it. Basically this means he's a better defender than attacker.

Do other characters go insane even if they have a [Terror] icon or the Willpower keyword?

No, only those who can go insane are affected.

To answer my own question: it is a response, not a forced response. So I guess she can just opt not to use it? 

Yes on both counts, so if no-one is there who will go insane, don't bother.
