What about if you marshal her as a duplicate?

No. Duplicates don't count. Istaril or Ktom will explain why :)

What about if she is in the initial setup and revealed?  My wife played her on the initial set up and I wasn't sure if that counted as marshalling her and triggering her ability/

Setup doesn't equal marshalled, played, or put into play. Its just set up. So you can't trigger her ability when you reveal your setup. Placing her on setup isn't ideal, but can work if you have another r'hllor card in hand to play turn 1

RRG, p. 20 ("Setup" Entry):

"9. Reveal setup cards. All players turn their setup cards faceup simultaneously. Then, each player attaches his or her attachments and places his or her duplicates, in player order. Note: Cards are not considered to have been marshaled, played, put into play, or to have entered play when they are revealed or announced in setup, and card abilities that reference those terms are ignored during this step. These cards are considered “already in play” when the game begins."

Does Mel trigger if you bring her in with Arianne. Initial thought is no since it says play or marshall. Since Ariane bring into play instead of play or marshall she wouldn't trigger.

Mel does not trigger in combination with Arianne's "put into play" effect for the reasons you say. There are a couple of threads on the rules board that go into the question in more detail if you are curious.

So the following situation arose last night with our play group: My opponent (first player) played Seen in Flames, while I had Bran and Robb on the board. Our doubt was whether Melisandre's kneel would trigger first, or whether Robb's stand would (should I decide to sacrifice Bran to cancel the event). In the end, we determined that after I sacrificed Bran, he had the first opportunity to trigger Mel. If he passed on that, and I activated the reaction from Robb, then the action would return to him, and he would be able to trigger Mel. However, if he passed, and I passed (which was what ended happening, since I only had one kneeling character), then the window to trigger reactions would close.

 

Did we resolve this correctly?

No. 

Bran is a would interrupt to the intitiation of the ability (Step 6), if used, he is sacrificed as a COST to initiating his own ability (Now, if we allow nesting, Step 6.4) . You're actually reacting to his sacrifice in 6.4, before his effects even interrupt the Seen in Flames.

Hmm, ok. I must say that feels a bit weird.

 

On one hand, Melisandre won't trigger until after Seen in Flames fully resolves, despite Step 6 saying "the card is marshalled/played", and Mel's text making reference to exactly that. So Mel has to wait for the effect to fully resolve before reacting.

 

On the other hand, Robb reacts to Bran's sacrifice (which would seem to happen in step 4), but doesn't have to wait for the effect to fully resolve to react.

 

Reading back on the Seen in Flames article, I guess this all comes back to the ruling that reactions to events being played are not triggered until after it fully resolves.
