If this appear, I evade a enemy costing 2 actions, can I still move to a adjacent location costing 1 action?

Yes. It is only the first time you perform one of those actions. It doesn't matter which, just the first of the 3.

Yes. It only penalizes the first ("one") of move, fight, or evade, not the first of each.
