Yes. You spend the click to play the Operation. Thus, it nets you 1 click.

Meadbeard is correct. 

 

A primary reason why this is useful is that you can play this on your 1st click to go to 4 clicks, and then install, advance, advance, advance to score a 3/2 agenda out of hand.

 

This and other various strategies that allow scoring out of hand are collectively called "fast-advance".

 

The reason why it is useful to score out of hand is you avoid leaving an agenda on the table during the runners turn, preventing the runner from making a run on the agenda and potentially stealing it during their turn.

If you have the money -- and a whole lot of luck -- you could play 3 of these out of your hand as the first 3 clicks of your turn, spending 12 credits but "gaining" 6 clicks (netting 3).

 

Then, you might put a 5/3 Agenda on the table and advance it 5 times to score it "out of hand," with the Runner never having a chance at it.

 

Now, before you get super-excited (I'm talking to the n00bster ;) , not KillerShrike, who already knows this), keep these two things in mind:

 

1) 4 of the 5 cards in your hand were very specific and the result of luck, which you can't control

 

2) that thing I just proposed cost a whopping 17 credits, all at once, which took a long time and a lot of effort to get into your bank

 

In other words, I'm pointing out to you what is possible (since you're new), but not what you should expect or what I think will ever happen.
