If she killed and cannot be saved can she become an attachement?
Her effect isn't a save, so yes.
So can there be more than one copy of her in play at a time if she is attached to a character? Does she just become a generic handmaiden attachment or keep her uniquiness?
The name of the card doesn't change. You cannot have a character and an attachment copy of her in play at the same time any more than you could have a character and an attachment copy of Grey Wind in play at the same time.
If a character has Irri as an attachment and dies, what happends with Irri? Goes to discard or dead pile?.
At that point, she is only considered to be an attachment. Attachments are discarded when the card they are attached to leaves play, and Irri goes to the discard pile, unless something else says otherwise.
