If a card like Confession (KotS) isn't paid for with influence, can it be cancelled?

Yes.
Is there any reason at all to not include this card in every deck?

It takes up an event slot and a lot of decks don't care about opponents events or they only really care about events that cost influence, or they have better cancels. It can be a good card but there are plenty of reasons not to include this.

It also varies a lot on play style, would you rather have the cancels for when your opponent tries something or would you rather be on the offensive and force your opponent to have the cancel when you try something.

I have to disagree, the event itself still has an influence cost, paper shield doesn't say " cancel an event played without influence" playing the event by dodging the cost doesn't negate that the event still has a cost.
The event's cost is what you pay to trigger it, not what you could have paid to trigger it. I'll try to find the ruling on this, but that's definitely how it works.

EDIT: discussed in detail in the comments section of this Quill & Tankard Regulars article.
Which one would you say is the best cancel between Paper Shield (QoD) and The Hand's Judgement (TTotH) ?

Depends on what you are expecting to face. For example, Paper Shield is quite useless against Martell Kothh which uses influences to pay for almost every event (except He Calls It Thinking). In that case, if you really want to cancel those events (and some others from Targ Kothh, for example), you'll need Hand's Judjment, but having to keep 1 gold kind of sucks...

If my opponent uses PS on my event can I use my own PS to cancel his and still have my event triggered?

Yep :)
