I actually kind of like this. The uniqueness is a downside, but I can see running x1 or x2 in some decks. It might work ok out of MaxX for instance. Not awesome, but playable.

Even with the brain damage its a net gain of +2 hand size. The question is, why take this over Public Sympathy? Simply because its Hardware so better protected?

Agreed.  If you can spare the card slots, that is.  If I'm going to devote deck space to +hand size cards, I'm usually going to lean towards the one that doesn't damage me!  At least Public sympathy won't leave me with a long-term problem if it gets trashed.

The main thing for this card over Public Sympathy is that hardware is generally more resilient. Though in pragmatic terms I've never seen or heard of anyone bothering to trash a Public Sympathy, it is in theory vulnerable to tag punishment. There is also hardware punishment of course, but it is still somewhat less common as it requires one or more cards for the corp while resource trashing via tag is a default action.

 

Additionally, as hardware it can be discounted or tutored via various other cards. For instance, Kate can install it for free rather than paying full cost for Public Sympathy, which is ironic given that she is the depicted and quoted character on Public Sympathy. 

 

 

I generally wouldn't consider playing both this and Public Sympathy; one or the other, and taking Public Sympathy over this card in most cases. However, if its status as hardware were more advantageous to my overall deck, I would consider using Brain Cage over Public Sympathy. More unlikely, if I were trying to do a cybernetics deck (which, frankly, seems non-viable overall), I would obviously favor this over Public Sympathy. It's all about context and taking advantage of synergies.
